Boxing is a sport with risk of injuries due to various activities involved. Please take some time to fill out a consent form before you start practicing with us.

Competitors and Volunteers

If you plan on competing, you must register through USA Boxing. Make sure to schedule a physical appointment with your doctor, because it will be needed for this form. Also, if you would like to become a volunteer, you must register on USA Boxing and pass a background check.
